STEP TWO:

Download this Reservation Form by clicking on it. Fill it out completely, and then complete Step Three.

STEP THREE:

Mail the filled out Reservation Form with your signature along with a check to: Illinois International Travel, Memory Maker Tours, 6724 Commonwealth Dr., Loves Park, Illinois 61111.

Please enclose $250.00 per person deposit for Tours. Make all checks or money orders payable in U.S. Dollars to : Illinois International Travel.

Be sure to enclose a copy of your REAL ID Driver’s License for all U.S. Bethel Tours and a copy of your valid PASSPORT (for the Canadian Tour) in order to receive a confirmation.
